Add Father's Details to a Birth Certificate

Who Can Apply

Only the biological parents of a child may add a father's details to a Birth Certificate.

Proof of Identification

Each parent must provide at least three (3) individual forms of identification from the list below with the application.

Identification List

  • At least one (1) form of identification must be provided from List 1 and must contain your full name, date of birth and place of birth.
  • The other forms of identification (at least two) must be provided from List 2 and/or List 3.
  • If you are unable to provide identification form List 2, you must provide at least two (2) forms of identification from List 3.

•  Australian Birth    Certificate

•  Australian Citizenship    Certificate or    Naturalisation    Certificate

•  Certificate of Evidence    of Resident Status

•  NZ Birth Certificate

•  NZ Citizenship    Certificate together with    passport

•  Australian Driver's    Licence (current)

•  Australian Passport    (current)

•  Firearms Licence    (current)

•  Foreign Passport    (current)

•  Medicare Card

•  Current Credit Card or    ATM Card

•  Centrelink or Veterans    Affairs Card

•  Security Card/Crowd    Control Licence

•  Tertiary Education    Institution ID Card

•  Australian Taxation    Office Group Certificate,    assessment notice or    advice

•  Council land rate or    water rate notices,    electricity or other utility    accounts with current    residential address

•  Defence Force or    Police Service photo ID    card

•  High School or    University Diplomas

(Please note that a Visa Endorsement in your passport is insufficient to confirm Permanent Resident Status).

Photocopies of current identification documents are accepted but they must be certified by a Qualified Witness as being 'true and correct copies' of the originals.

A Qualified Witness is a:

  • Justice of the Peace
  • Notary Public
  • Barrister/Solicitor
  • Commissioner for Affidavits
  • Officers at the NSW Registry of Birth Deaths & Marriages

The Qualified Witness must provide their name, address and daytime telephone number on the Statutory Declaration.

The Registry will not accept faxed applications or faxed identification documents.

*All certificates and Birth Cards are distributed through Registered post (except Family History) and incur a $5.00 (inc. GST) postage and handling fee. Prices listed are inclusive of the postage and handling fee. Registered post does not ensure next day delivery.

Your Adding Father's Details to a Birth Record application must include:

  • Statutory Declaration form and,
  • Payment Details form and
  • At least three forms of identification from each parent

Identification must be included with your application.

The application forms can be completed on your computer, printed and then mailed to the Registry.

Please note that you are unable to submit your application online and the Registry will not accept faxed application forms or faxed identification documents.
