Who Can Apply
Only the biological parents of a child may add a father's details to a Birth Certificate.
Proof of Identification
Each parent must provide at least three (3) individual forms of identification from the list below with the application.
At least one (1) form of identification must be provided from Category 1 and must contain your full name, date of birth and place of birth.
The other forms of identification (at least two) must be provided from Category 2 and 3, or 3 and 4.

Please note that a Visa Endorsement in your passport is insufficient to confirm Permanent Resident Status.
Photocopies must be must be certified as true copies of the original by a Qualified Witness (e.g. Justice of the Peace; Notary Public; Legal Practitioner with current Practising Certificate; person authorised to administer an Oath under S.26 of the Oaths Act 1900).
The Qualified Witness must provide their name, address and daytime telephone number on the Statutory Declaration.
