Add a parent to a birth

For a birth that occurred in NSW, you can apply by post or in person to:

The form to add a parent can be used to: 
  • Add a father's details to a birth.
    The Registry allows an addition to details on a Birth Certificate when you wish to add the biological father's details, or:
  • Add a mother's details to a birth.
    The Miscellaneous Act Amendment (Same Sex Relationships) Act came into effect on 17 September 2008. Subject to the requirements of the Act, both women in a same sex de facto relationship may now be recognised on their child’s birth certificate.

The Registry accepts postal applications. Mail the completed application with your payment details and certified copies of identification.

You can also apply in person at a Registry Office. No appointment required at Chippendale and Parramatta.
