If you have already changed your name or your
child's name and you require a replacement copy of the certificate,
you can apply for a Replacement Change of Name Certificate.
If you are the parent of a child over the age of 18, you
are entitled to the replacement certificate if you applied
for the original change of name.
- Please provide at least three (3) forms of your identification
with your application.
If you are not the person named
on the certificate or you did not
apply for the original change of name, you must satisfy one
of the following conditions:
- Provide written authority AND identification
from the person named on the certificate or one of their
parents OR
- Show Power of Attorney relating to the person named on
the certificate OR
- Be a solicitor/welfare group acting for the person named
on the certificate.
