If you were born in NSW |
- You will receive either a Birth Certificate or
a Change of Name Certificate. If you wish to purchase
both, please add $39.00 to your fee.
- If you already have a NSW Birth Certificate or
a NSW Change of Name Certificate and you wish to have your change
of name recorded on your birth registration, you must return the original
certificate/s, including any extracts you may have.
- Your birth registration will be amended and will
include details of any previous names used.

If you were born in Australia but not in NSW |
- You will receive a NSW Change of Name Certificate.
The Registry in the state you were born in may be
notified of your change of name registration.
- If you have previously registered a Change of Name
with the NSW Registry, you must return your Change
of Name Certificate with your application.

If you were born overseas |
- You will receive a NSW Change of Name Certificate.
If you have previously registered a Change of Name
with the NSW Registry, you must return your Change
of Name Certificate with your application.

If your child was born in NSW |
- You will receive either a Birth Certificate or a
Change of Name Certificate. If you wish to purchase
both, please add $39.00 to your fee.
- If your child already has a NSW Birth Certificate
or a NSW Change of Name Certificate and you wish to
change the child's name, you must
return the original certificate/s (including any birth
extracts).
- The child's birth registration will be amended
to include change of name. The parents' names
are not changed.
