Change of Name Interviews

As of Monday 2 March 2009, customers wishing to register a change of name in person at the either Chippendale or Parramatta offices will need to make an appointment. To make an appointment for a change of name interview, call the Registry on 1300 655 236. Alternatively, the Registry continues to accept Change of Name applications by mail.

Your Change of Name Application must include:

• Statutory Declaration form and,
• Payment Details form and,
• At least three forms of your identification, and
• A NSW Birth Certificate or a NSW Change of Name Certificate (if you have one)

Identification must be included with your application.

Please print and complete the application form, then mail it to the Registry.
