Routine birth, death and marriage certificate applications may take up to 3 weeks. Urgent applications are currently taking 5 working days to process.

New registrations are taking (birth 3 weeks, marriage 4 weeks & Change of Name applications 7 weeks) to process, including the issue of your certificates.

The Registry apologises for any inconvenience.

Your Change of Name Application must include:

• Statutory Declaration form and,
• Payment Details form and,
• At least three forms of your identification, and
• A NSW Birth Certificate or a NSW Change of Name Certificate (if you have one)

Identification must be included with your application.

The application forms can be completed on your computer, printed and then mailed to the Registry.
