Amend Birth Certificate

Details of a biological father can be added to a Birth Certificate at any time. A fee will apply for this service, which includes the issue of a Birth Certificate. This process is called Adding a Father's Details to a Birth Record.

You may also amend your child's given name if they are under three (3) months old and their birth has been registered in New South Wales. There is no charge for this amendment, however you will need to purchase a new Birth Certificate for your child. This process is called Amending the Given Name of a Child Under Three Months Old. This procedure is only available where the child's name has been misspelled or a middle name was omitted from the birth registration form. The parents are required to provide evidence that the correct spelling or full name intended for the child has already been used by providing one (1) or more of the following:

  • Baptismal Certificate
  • Bank book or Statement Account
  • Child's passport
  • Medicare Card
  • Family payment letter/advice
  • Letter from child's medical practitioner confirming correct name

Please note that parents are required to complete a Statutory Declaration form and a Payment Details form for adding a father's details to a birth record and amending the given name of a child under three months old.

The Registry will not accept faxed applications or faxed identification documents.
