To apply for a change of sex on your Birth Certificate you must:

• Be born in NSW
• Be over 18 years old
• Not be married
• Have undergone sexual reassignment surgery

Your application form must be accompanied by statutory declarations from two (2) Medical Practitioners, confirming that the sexual reassignment surgery has occurred.
