Proof of Identification

You must provide proof of identification with your application for a Change of Sex.

Please note that your application must also be accompanied with two statutory declarations from two separate Medical Practitioners confirming that the sexual reassignment surgery has occurred.

Please provide at least three (3) forms of identification from the list below with your application.

  • At least one (1) form of identification must be provided from each list in the table below.
  • If you are unable to provide identification from List 1, you must provide at least two (2) forms of identification from List 2 and at least one (1) form of identification from List 3.

•  Australian Driver’s Licence
•  Medicare Card • Council land rate or water    notices, electricity or other    utility accounts with    current residential address
•  Australian Passport • Currrent Credit or ATM Card • Bank statement with   current residential address
•  Firearms Licence • Centrelink or Veterans    Affairs Card  
•  NSW Birth Certificate •  Security Guard / Crowd    Control Licence  
  •  Tertiary Education    Institution ID Card  

All identification must be current.

Photocopies of identification are accepted, but must be certified by a Qualified Witness as being 'true and correct copies' of the originals.

A Qualified Witness is a:

  • Justice of the Peace
  • Barrister/Solicitor
  • Commissioner for Affidavits
  • Notary Public
  • Officers at the NSW Registry of Births Deaths & Marriages
