Recording a change of sex on a Birth Certificate requires particular documentation including:

1. Applicant's Statutory Declaration and,
2. Supporting Statutory Declarations (x 2), completed and signed by two Medical Practitioners verifying that you have undergone sexual reassignment surgery and,
3. At least three forms of the applicant's identification and
4. Payment details form

You can complete the application forms on your computer, then print them and mail to the Registry or deliver them to a Registry office in person.
