This application can only be used to correct
the given name/s of a child under three months old, whose
birth is registered in NSW.
Who Can Apply
Only the parent/s of the child may apply
to amend the given name of a child. The child must be under
three months old at the time of the application.
Consent of both parents (recorded on the child's Birth Certificate)
is required to amend a child's given name. If the details
of a father are not shown on the Birth Certificate, only the
mother's consent is required.
Proof of Identification
Each parent must provide at least three (3) individual forms
of identification from the list
below and at least one form of their child's
identification.
Child's Identification
Parents must provide at least one (1) form
of their child's identification from this list. The identification
must include the correct spelling of the
child's name.
- Baptismal Certificate
- Bank book or Statement Account
- Child's passport
- Medicare Card
- Family payment letter/advice
- Letter from child's medical practitioner confirming correct
name
If you have a Birth Certificate for your child, please attach
it with your application.
Identification List
Each parent must provide at least three (3) individual forms
of identification from the list below with their application.
- At least one (1) form of identification must
be provided form List 1 and must contain
your full name, date of birth and place of birth.
- The other forms of identification (at least two) must
be provided from List 2 and List 3.
- If you are unable to provide identification form List
2, you must provide at least two (2) forms of identification
from List 3.

(Please note that a Visa Endorsement in your passport is
insufficient to confirm Permanent Resident Status).
Photocopies of current identification documents are accepted
but they must be certified by a Qualified Witness
as being 'true and correct copies' of the originals.
A Qualified Witnesses is a:
- Justice of the Peace
- Notary Public
- Barrister/Solicitor
- Commissioner for Affidavits
- Officers at the NSW Registry of Birth Deaths & Marriages
The Qualified Witness must provide their name, address and
daytime telephone number on the Statutory Declaration.
The Registry will not accept faxed applications or faxed
identification documents.

Please note there is no cost to amend the given name of a
child under 3 months old. However, you will need to purchase
a Birth Certificate for your child. Along with an official Standard Birth Certificate, the Registry
also offers a range of Commemorative Birth Certificates. All
Commemorative Birth Certificate packages include a Standard
Birth Certificate.

- The Commemorative Certificate is only issued for commemorative purposes and is not intended for official use.

Delivery
- Urgent applications lodged in person at the Registry office
are ready for collection after 24 hours.
- Urgent mail applications will be sent by secure post
the next working day after they are received and will incur a postage and handling fee.
- Non-urgent mail applications are sent by secure post five (5)
working days after they are received and incur a postage and handling fee.
- Non-urgent applications lodged in person at the Registry
office are ready for collection in five (5) working days.
The times stated above begin from the receipt of the application
at the Registry until the certificate leaves the Registry's
premises. If your application is incomplete, the times stated
do not begin until your application complies with the Registry's
requirements.
