• Adding a Parent's Details to a Birth
  

This form can be used for:

• Adding a Father's Details
  The Registry allows an addition to details on a Birth Certificate when you wish to add the biological father's details.

OR

• Adding a Mother's Details
  The Miscellaneous Act Amendment (Same Sex Relationships) Act came into effect on 17 September 2008. Subject to the requirements of the Act, both women in a same sex de facto relationship may now be recognised on their child’s birth certificate.

Print and complete the application form, and mail to the Registry with payment details and certified copies of identification.
