| The Registry adheres to strict proof of identity
guidelines when issuing certificates.
All applications must be accompanied with identification.
If you do not provide the appropriate identification, your
application will not be processed.
Birth, Death and Marriage Certificates
If you are applying for your own certificate, you must provide:
- At least three (3) forms of identification from the list
below with your application.
If you are applying for a certificate on behalf of someone
else, you must provide:
- At least three (3) forms of your own identification from
the list below and,
- At least three (3) forms of identification from the list
below from the person named on the certificate and,
- A letter giving permission from the person named on the
certificate. The letter must include their address, daytime
telephone number and signature.
Identification
You must provide at least three (3) forms of identification
from the list below with your application.

At least one (1) form of identification must be provided from Category 1 and must contain your full name, date of birth and place of birth.

The other forms of identification (at least two) must be provided from Category 2 and 3, or 3 and 4.
All documents, except foreign passports, must be current.

Changes of Name and Amending Certificates
If you are applying for a Change of Name or wish to amend
a certificate, please refer to the relevant section on
the website as the identification requirements differ based
on the application.
Please note that at least three (3) forms of identification
are required with each application.
Photocopies of identification must be certified by a Qualified
Witness.
Qualified Witnesses include:
- Justice of the Peace
- Notary Publics
- Solicitors
- Barristers
- Officers of the NSW Registry of Births Deaths & Marriages
- Commissioner for Affidavits
If you do not provide the appropriate identification, your
