The Registry issues Replacement Change of Name Certificates for changes of name which have been completed in NSW after 1 April 1996.

If the change of name was registered in another state or territory of Australia, please contact the relevant Registry Office within that state or territory.

Before applying for a Replacement Change of Name Certificate, you need to check if you can apply for it.

All Replacement Change of Name applications must be accompanied by at least three forms of identification. This ensures your privacy is maintained and that information is only released to those who are entitled to it.
