Register Change of Name

The Registry records official changes of name for adults and children (under 18 years).

You are eligible to have your name changed in NSW if:

  • Your birth is registered in NSW or
  • You were born overseas and have been resident in NSW for three (3) consecutive years immediately prior to your application or
  • Your birth is not registered in NSW and a protection order has been made to protect you and/or your children from domestic violence.

The Land Titles Office no longer registers Deed Polls or Instruments Evidencing Change of Name.

If a person alters their name or uses an additional or other name with the intention to act fraudulently or with an intention to deceive or in any other way that contravenes the law, they may be subject to criminal proceedings.

Change of Name Interviews

  • To organise a Change of Name interview, please phone 1300 655 236 to arrange an appointment. Alternatively, the Registry accepts applications by mail.

Your Application must include:

  • Statutory Declaration form and,
  • Payment Details form and,
  • At least three forms of your identification, and
  • your original certificates
    (NSW Birth Certificate / Change of Name Certificate)

Include all required Identification and mail to:

  • NSW Registry of Births Deaths & Marriages

Certificates

  • Change of Name includes registration fee and either
    a Birth or Change of Name Certificate.
  • You must return all of your original certificates with your application including any NSW Birth or NSW Change of Name Certificates and extracts.

  • You receive a Birth Certificate or a Change of Name Certificate.
  • To purchase both, please add $40.00 to your fee.
  • Your birth registration will be amended and will include details of any previous names used.
If born overseas
  • You receive a NSW Change of Name Certificate.

If your child was born in NSW
  • You receive a Birth Certificate or a Change of Name Certificate.
  • To purchase both, please add $40.00 to your fee.
  • The child's birth registration will be amended to include change of name. Parents' names are not changed.
If child born overseas
  • You receive a NSW Change of Name Certificate.

Cost

Cost for a Change of Name for an adult and child are the same:

Urgent applications                   $198.00

Non-urgent applications            $154.00

Certificates are distributed through registered post and prices listed are inclusive of a postage and handling fee. Registered post does not ensure next day delivery.
