From 2 March 2009 to register a change of name in person at either Chippendale or Parramatta offices you need to make an appointment. For a change of name interview contact us. Alternatively, we still process non urgent Change of Name applications by mail.

From 24 November 2008 we don't process urgent Change of Name applications over the counter or via the post.

Your Change of Name Application must include:

• Statutory Declaration form and,
• Payment Details form and,
• At least three forms of your identification, and
• your original certificates
  (NSW Birth Certificate / Change of Name Certificate)

Change of Name Application Forms:

• Adult Change of Name (18 years +)
• Child Change of Name (under 18 years)    

Identification must be included with your application.
