with your application.
Your application must also be accompanied
with two statutory declarations from two separate Medical
Practitioners (registered in Australia) confirming that the sex affirmation procedure
has occurred.
If your birth is registered in NSW
Please provide at least three (3) forms of identification, from the list below (one of each from Categories 1, 2 and 3 or 4) with your application.
- If you are unable to provide identification from Categories 1 and 2, you must still provide at least three (3) forms of identification. At least two (2) of these must be from Category 3.
- One (1) form of identification must show your current residential address.

- If your Category 1 document does not include your place of birth, you must also provide a passport or full birth certificate containing this information.
- At least one (1) form of identification must contain your current residential address.
- Your Category 4 document must show that you have resided in NSW for the past 12 months.

If your application is lodged by mail, photocopies of identification are accepted, but must be certified
by a Qualified Witness as being 'true and correct copies'
of the originals.
A Qualified Witness is a:
- Justice of the Peace
- Legal Practitioner (holding a current Practising Certificate)
- A person authorised to administer an oath under section 26 of the Oaths Act 1900
- Notary Public
