Recording a change of sex on a Birth Certificate and Registering a Change of Sex requires particular documentation including:

1. Applicant's Statutory Declaration and,
2. Supporting Statutory Declarations (x 2), completed and signed by two Medical Practitioners verifying that you have undergone a sex affirmation procedure and,
3. At least three forms of the applicant's identification and,
4. Payment details form

Please print and complete the application form, then mail to the Registry or deliver them to a Registry office in person along with your identification.

Identification must be included with your application.
